Weight loss is associated with a significant reduction in BMI and waist circumference, and a greater risk for developing health conditions such as type 2 diabetes, hypertension, and high cholesterol.
A small, randomized controlled trial (N=664) followed participants for 6 years. Participants reported their weight and their adherence to diet and exercise regimens was evaluated.
Adherence was measured by the number of meals a day, the number of times participants took the medicine, and by their body mass index (BMI). Participants were asked about their weight loss over the 6 years.
Women: Those with a BMI of over 25, or a BMI of under 30, were eligible if they had a body mass index (BMI) of at least 28 kg/m2 or a body mass index (BMI) of over 15 kg/m2.
Men: Those with a BMI of 25 to 29 kg/m2 or a BMI of greater than 30 kg/m2, respectively, were excluded from the trial.
Participants were randomized in a 3:1 ratio to one of diet, exercise, or placebo groups. Following the first 3 weeks of treatment, participants were randomly assigned to one of the following 3 groups: a dummy treatment; or a treatment with orlistat, a prescription weight loss medication, or a placebo.
The diet and exercise groups were started on a low-fat diet, and participants received regular weekly exercise. The weight loss groups were started on a low-fat diet, and participants received regular weekly weight loss medication. Following 6 months, participants in the treatment with orlistat group were randomized to receive either a placebo or a treatment with orlistat. In the treatment with orlistat group, participants were assigned to receive either a placebo or a treatment with orlistat.
Adherence to the study diet and exercise regimen.
Primary outcome: Adherence to the study exercise regimen.
Secondary outcomes: Body mass index, waist circumference, mean weight, and mean BMI.
Adverse events were monitored by the Investigator, and adverse events were categorized into grade 3, 4, or more adverse events based on the Investigator’s rating of the adverse events. Grade 3 adverse events were defined as requiring immediate medical attention, including severe, persistent, or bloody diarrhea, nausea, vomiting, and rash. Serious adverse events included new-onset seizures, myalgia, acute pain, abdominal pain, hypotension (blood pressure of more than 300/80 mmHg), dyspnea, and peripheral edema.
The study medication was given orally by intravenous injection every 2 weeks. Participants completed the 6-year study period for the weight loss, and participants were randomized 1:1 to orlistat or a treatment with orlistat (the dummy group). The treatment with orlistat (the treatment with orlistat) was stopped in the event that participants lost ≥ 5% of their body weight. Participants in the treatment with orlistat group were also randomly assigned to take either a placebo or a treatment with orlistat.

Orlistat is used to treat obesity and help in weight loss. Orlistat is to be used along with a low-calorie diet and regular physical exercise.
